Mute That Actually Mutes
There are three places a microphone can be silenced and only one of them is certain. Why the button you press is usually a request, and how to test which you have.

Record yourself while muted, in a different application, and listen back. That two-minute test is the only way to know which kind of mute you have, and most people have assumed the answer for years.

The problem, as it appears
You were muted. The icon said so. Someone tells you afterwards that they heard the whole thing.
Or the reverse: you press mute, speak freely, and later discover the other application — the one recording, or the one you forgot was open — had the microphone the whole time.
Both come from the same misunderstanding, which is that "mute" is one thing.
Why the icon disappoints
Because it is telling the truth about a smaller domain than you assumed.
A mute control inside a call application is a setting inside that application. It is accurate, it works, and its authority ends at that application's boundary. Anything else with the microphone open is unaffected, because nothing told it anything.
That is not a bug and it is not deceptive. It is simply a narrower promise than the word suggests, and the interface has no way to show you the scope of what it controls.
What actually matters
There are three places to silence a microphone
Worth separating, because they behave differently under pressure.
In the application. The call software stops sending your audio. Scoped to that application only. This is what almost everyone means by mute and it is the weakest of the three.
In the operating system. The system mixer mutes the input device. Broader — it affects applications that respect the mixer, which is most but not reliably all of them.
At the hardware. The signal path is interrupted at the device. Nothing downstream can capture what is not being passed, so no software has any say in it. This is the one that cannot be overridden.
Your headset button is probably not hardware mute
An inline control on an analogue headset frequently does break the circuit — a physical switch in the cable. That is genuine hardware mute.
A button on a USB headset generally is not. It typically sends a command, either to the device's own firmware or as a keystroke the computer interprets, and both of those are software with a physical button on the front.
Neither is bad. The problem is only that they look identical and behave differently in the one situation where it matters.
The test is the recording, not the indicator
Open a recorder in a separate application. Mute yourself the way you normally do. Talk. Stop the recording and listen.
If your voice is on it, your normal mute is application-scoped. That is worth knowing before it matters rather than after, and no amount of reading a specification substitutes for it.
Push-to-talk removes the failure mode entirely
The reason push-to-talk is worth the adjustment is not that it is more reliable in a technical sense. It is that it inverts the default: silence is what happens when you do nothing.
Every mute failure has the same shape — you believed a state was active and it was not. Push-to-talk makes the dangerous state the one requiring continuous effort, so forgetting produces silence rather than broadcast.
It also helps with a problem covered elsewhere: a microphone that is only live while you are talking is not picking up your keyboard for the rest of the meeting — stopping keyboard noise reaching your mic.
Unmute has a delay and it eats your first word
Several layers can be involved, and some ramp rather than switch. A noise gate re-opening, a stream resuming, a device waking — any of them can swallow the first syllable.
The practical fix is habit rather than equipment: unmute, then speak, in that order, with a beat between.

Learning what your existing mute does suits everyone and costs nothing. Do the recording test before buying anything, because the answer may be that you already have hardware mute and did not know.

A volume knob with a mute function suits a desk that wants one physical control for several things. Same caveat: confirm what the mute is actually doing.

The mistake to avoid
Trusting an indicator whose scope you have never checked. The icon is honest about the layer it controls, and the layer it controls is usually narrower than the word implies. If you have never tested it, you do not know which of the three you have, and the moment you find out is not one you get to choose.
The second mistake is muting in the application when you meant to stop being heard by everything. Application mute is the right control for a meeting and the wrong one for privacy — for the second, the answer is hardware, and the honest version of that is a switch you can see is open.
